This form is used to determine your preliminary eligibility for the affordable apartments.

Important Notice

All information you provide must match your most recently filed tax return (income and household details). Any mismatch will result in rejection without further review or discussion.

  • Ensure you have a credit score of 575 or greater before applying.
  • Upload income proof that matches the household gross income you enter below.
